Mother Stabbed to Death 16 Days After Warning Police of Threat
A woman warned officers that her estranged husband posed a serious risk. Despite reports that he threatened to "cut her head off," police rated her danger as only "medium." A Domestic Homicide Review later found that the threats and controlling behaviour were not taken seriously. Sixteen days after her ex-partner’s arrest, he entered her workplace and fatally stabbed her before taking his own life. The review highlighted failings in the follow-up risk assessment and noted that bail conditions did not prevent him from being at her studio. Her brother demands answers and accountability. He says a proper risk evaluation might have saved her life and criticises the police for treating her as just another tick-box case. The force now offers enhanced training on coercive control, but family members still seek responsibility for the tragedy.
